If you are 100% sure the cause of your bad breath is not coming from your teeth and or gums, then try talking to your doctor for advise.
Some times bad breath can be cause by
1) Sinus conditions
2) Smoking
3) Dry mouth
4) Other Health Issues
5) Think about having a consultation with a medical doctor. Your doctor will of course know what types of conditions to look for but, in general, they will look for problems associated with the respiratory (pulmonary or bronchial), hepatic (liver), renal (kidney), and gastrointestinal (stomach and intestine) systems.

I know just how you feel. I too suffer from really bad breath. Just like you my breath had nothing to do with my teeth or gums being bad. For me it was a sinus condition.
I have really bad sinuses so it effective my breath. Knowing this help me control my bad breath because I knew when my sinus was acting up so would my bad breath so I had mints on me during that time.

I would check all the possible reason for you bad breath. Go to the doctor have him look into it. See you dentist again just to make sure. In the mean time try chewing sugar free gum.
