I brush after every meal and snack so that mean I brush at least four times a day. I also floss at least that many times a day as well. I use mouthwash with germ killing, so I just do not get it.

Go to your doctor. You could have a health related problem that is effecting your teeth and gums. One such example would be diabetes. I am not saying you have that but diabetes can effect teeth and gum health.
At least have your doctor in the loop to checks things out for you.

Well talk to your dentist and see about having sealants put on your teeth. I will not help the receding gums but can help you on the cavity issues.
My mom has the receding gums issues and one of the things her dentist dose is have her come in for clean and maintenance four times a year instead of just two. You may want to start doing that as well.
